Insurance card (for those with dental insurance)
Please send our office copies of any previous dental records including x rays, if applicable

Finances & Insurance

We believe in dental freedom with uncompromising care. That’s why we treat you based on your personal needs, not your insurance plan. Without the restrictions of insurance, we can deliver everything you need and exactly what you want. If you do have dental benefits, we will gladly submit claims on your behalf to help you maximize your dental benefits. Payment in full is required for services rendered at the time of treatment. For financing, we do offer CareCredit and in-house options that allow you to pay over time.
